What is the ADVANTAGE Medication Therapy Management Program?

Medication Therapy Management, often abbreviated as MTMP or simply MTM is a program where a wide range of healthcare services are provided by pharmacists, nurses and other health care providers to ensure you get the most benefit from your prescription drug therapy. Please note that MTMP is not a part of your benefit, but is instead an added service available and provided at no additional cost to the member.

Who is eligible for MTMP?

Generally speaking, in order to be eligible for the 2010 MTMP program, you must have three (3) or more of the following chronic medical conditions: Alzheimer’s, Arthritis, Asthma, Bipolar Disorder, Chronic Heart Failure, Depression, Diabetes, Heart Disease, Osteoporosis or Schizophrenia. You must also be taking more than 8 medications and incur significant expenditure for medications.

Can I enroll in the MTMP program?

Members will be identified through the use of prescription records and automatically enrolled into the program if they meet the criteria. Members will be identified on a quarterly basis using the previous three (3) months of pharmacy claims information.

Do I have to participate in the MTMP program?

No. Participation is voluntary on an opt-out basis. If you are identified as a member who meets the criteria, you will be contacted. At that time, you have the right to opt-out simply by telling ADVANTAGE that you do not wish to participate.

What will MTMP consist of?

MTMP will include a comprehensive review of your medication history, as well as possible interactions on the phone with medical personnel, and/or written information to assist you in getting the most from your prescription therapy.
