How can I find a participating network provider?

If you have the PHCS logo on your ID card, click on the logo to the right to find a particiapting network hospital: PHCS Member Hospital Directories
If you have the CHA/EMO/LCI logo on your ID card, click on the logo to the right to find a particiapting network hospital. ID cards with this logo are not valid until 7/1/2010. Please do not present this ID card to your provider until on or after 7/1/2010. CHA/EMO/LIC Member Hospital Directories

What if my doctor is not in the network?

If your doctor is currently not participating in the network, you may nominate your physician for consideration. By completing the Physician Nomination Form and following the instructions therein, the form will be automatically sent to ADVANTAGE for review. We will make every effort to follow-up on your request and respond back to you as soon as possible.
